Welcome to Creative Care Options (CCO) of Fond du Lac County.
We began providing long term care to people with developmental and physical disabilities and the frail elderly in 2000 and our goal has always remained the same—to provide high quality care in a cost effective manner to people who need assistance in remaining as independent as possible and to remain active and connected to their community.
To reach our goal, we have always focused on our members and their outcomes. We employ a compassionate staff of case managers and nurses to assist our members in developing highly individualized case plans tailored to meet the specific needs of our members.
A network of over 200 providers is available to meet the identified outcomes of the close to one thousand people we are proud to call the members of Creative Care Options.
The Family Care Program began as a pilot program in five Wisconsin counties in the year 2000. To learn more about the pilot program, see our history. Fond du Lac County was the first of the pilot programs to begin enrolling members. Both the Family Care Program and Creative Care Options have grown over the past seven years.
Creative Care Options is a fully operational county department functioning as an enterprise for Fond du Lac County. Family Care has grown out of the pilot stage and is being targeted for statewide implementation in the next five years.
