HealthCheck (Wisconsin’s EPSDT Program) is a preventive health check-up program for anyone under the age of 21 who has a valid Wisconsin Medicaid card (T19, Medical Assistance).
HealthCheck teaches you and your child how to prevent illness, and can also identify health problems early, before they become serious (remember a healthy child is a happy child!). HealthCheck meets the physical exam requirements for programs such as Head Start, Child Care, WIC (Women, Infants and Children Supplemental Food Program), and school physicals. A HealthCheck referral may also make you eligible for some services not normally paid for by Medicaid. Appointments can be arranged by calling the Health Department.
