The Aging and Disability Resource Center provides Information on a wide range of resources regardless of income and assets. We provide counseling to help people plan for their future long term care needs. We have Power of Attorney forms for healthcare and finances.
We determine eligibility for the Family Care Program which funds services for the elderly and adults with disabilities which are designed to keep people in the least restrictive setting possible. There is a Benefit Specialist on staff at the Aging and Disability Resource Center who provides assistance with application and appeals for Social Security Disability Programs.
We assist people with low income and assets with guardianships and protective placements.
Calls to report Elderly Abuse and Neglect situations can be made to the ADRC by calling 920-929-3466 or the main DSS phone number 920-929-3400. For Elderly Abuse and Neglect urgent calls after normal work hours, please contact your local law enforcement agency.

The Aging and Disability Resource Center services frail elders and people with physical and developmental disabilities.
Why would I contact the Aging and Disability resource Center?
The Aging and disability resource center staff assists the general public in navigating a complex service system. They can prevent or delay the need for the long-term care system, through appropriate referral to services that the person wants or needs. You will get the information that you need to make an informed choice about what is available and get linked to services and programs that provide services.
