PASSPORT REQUIREMENTS

1. The Passport application must be filled out in black ink or typed.  Do not use correction fluid or white out.

2. All applicants must appear in person, including children.  For children under 16 years of age BOTH parents must be present.

3. Certified copy of your birth certificate/it must have a raised seal.  A photocopy of the birth certificate should also accompany the application.  The photocopy is not mandatory but your passport may be delayed.

4.  Passport photo. This can be taken at Walgreens or Wal-Mart.

5. Copy of front and back of drivers' license, on one sheet of paper.  For minors, we will need a copy of both parents' drivers' license. 

Fees made payable to the US Department of State-must be personal check or money order

No credit cards or cash is accepted for this fee 

     Adult- $130.00 Passport Book                               Adult-$30.00 Passport Card

     Child 16 & over-$130.00 Passport Book               Child-$15.00 Passport Card 

     Children 15 & younger - $100.00 Passport Book

Fees to the Clerk of Courts Office-can be check, money order or cash-All applicants-$35.00
