The Fond du Lac County Department of Human Services Clinical Services Division is located at the Health Care Center, 459 E. 1st Street, Fond du Lac.

INPATIENT HOSPITALIZATION

Fond du Lac County residents are generally screened through our Crisis Intervention services. Out of county residents must go through the Crisis Intervention service in their county of residence for approval of admission.

Eligibility
- Fond du Lac County residency (Must provide one of the following)
- Verification of address through a utility bill in the last 90 days
- Bank, credit union, credit card, or mortgage statement.
- Current and valid WI driver’s license or State ID card
- Paycheck/stub.

Ability-To-Pay qualifications
- As the public SUD and mental health services provider, DHS offers services to individuals who are uninsured/underinsured, or on certain kinds of Medical Assistance benefits.
- A limited number of private insurance companies contract as “preferred providers” with us.
Helpful Hint: If you do have any kind of insurance, including Medical Assistance HMO insurance, it is important for you to contact your insurance company to see if DHS is a “preferred provider” under your coverage. If not, you will need to go to the preferred provider that your insurance company has identified for your services.
- Dial the Intake Phone number to reach the Intake desk.
- You will be asked first if you are a Fond du Lac County resident and to confirm your qualifying address.
- The Intake staff will then ask you to come in to the facility during the hours listed above to complete necessary paperwork which includes the following:
- Information sheet
- HIPAA form
- Primary Care and Physician forms
- Any other release forms for other agencies, previous treatment providers, probation/parole officer, court, etc.
- Ability-to-Pay form
- DHS is required by State law to do “Payment Level Determinations” using the State of Wisconsin Uniform Fee System. See their website.
- A member of our clinical staff will review your information to make sure you are seen by an appropriate clinician.
- You will receive a telephone call from the Appointment Desk staff to schedule your appointment.
