Comprehensive Community Services (CCS)

The Comprehensive Community Services Program is new to the Fond du Lac County Department of Community Programs. CCS is the latest administrative rule created by the State of Wisconsin Department of Health and Family Services for persons with severe and persistent mental illness and/or substance abuse addiction. DCP received licensure in September, 2006. CCS in our county will eventually serve all ages, adults first, then elders, then children with Severe Emotional Disorder (SED) diagnosis. The goal of CCS is to provide psychosocial rehabilitation services to consumers to reduce symptoms and restore consumers to their optimum level of functioning stability and independence.

Eligibility Requirements:

1) Medical Assistance benefits

2) A diagnosis of severe and persistent mental illness

3) Need of ongoing services due to an impairment that interferes with

or limits a major life activity.

4) Found functionally eligible through a state screening process.

CCS operates on the mental health “Recovery Model,” is based on collaboration, on “doing more of what works and less of what doesn’t”, and is consumer-centered as well as strength- and outcome-based.

Services and goals are similar to those provided under our Comprehensive Community Services program, but expand to embrace the recovery concepts of empowerment: hope, rights, roles, responsibilities, dreams, goals and decisions, identified by the consumer, and facilitated by the Recovery Team.
