  • Chlamydia
  • Gonorrhea
• Free condoms
• Testing is by appointment only and available to all ages and is completely confidential. 
  • Call 920-929-3085 to schedule, Monday-Friday 8am-4pm (Excluding legal holidays).

• We offer services on an income-based sliding fee scale
• Cost includes office visit for testing and treatment
• Cost will be determined at the time of the appointment

  • If yes, have you had more than one partner in the last 12 months?
  • Have you had sex with a partner who has had multiple partners?
  • Have you had unprotected sex in the past year?
• Would you like to be screened for chlamydia and gonorrhea?
• Are you interested in more information about STIs?

Facts about Sexually Transmitted Infections

• Most sexually transmitted infections (STIs) have no symptoms
• If left untreated, STIs can lead to problems getting pregnant for women and men; as well as other complications
• Annual screening is recommended for STIs
  • Routine screening is helpful in identifying cases that have no signs or symptoms
• STIs can spread through vaginal, anal, or oral sex
• People can lower their chances for STIs by:
  • Using condoms for every sexual act (including oral sex)
  • Limiting sexual partners
  • Both partners getting tested prior to having sex
