How to Apply for WIC
To find out more or to schedule an appointment:
- WIC is available Monday - Thursday. Call WIC at 920-929-3104 or stop by the WIC office at 160 S. Macy Street, 3rd Floor, Fond du Lac, WI.
A pre-screening application can be completed over the phone. Please have the amount of your gross income available.
To complete the WIC enrollment, you will need to bring or submit via the WIC portal:
- Identification for you and/or your children
- Proof of income
- Proof of residency
At your appointment, a WIC nutritionist will review your or your child’s health history. You will also discuss what you or your children eat and a health screening will include current height, weight and hemoglobin.
