FDL County Care Clinic

The Fond du Lac County Care Clinic is a benefit available to all County employees and family members who carry our Health Insurance Plan. It is a clinic where primary care services are provided by Agnesian Healthcare/SSM Health, located at 430 E. Division St., Fond du Lac, WI 54935. Cynthia Liethen, a Nurse Practitioner, is dedicated to the clinic and is assisted by support staff in providing services to insured members.

The hours for the clinic are: Mondays, Tuesdays, Wednesdays, and Fridays from 7 am to 5 pm. The phone number to schedule an appointment is: 920-926-5676.

**Fond du Lac County Care Clinic utilization on weekends is now available at the SSM Health Express Clinic, located at 145 North Main St, Fond du Lac. There are no office visit charges during this extension of Fond du Lac County Care Clinic services on Saturdays and Sundays.
