ADRCs are the first place to go to get accurate, unbiased information on all aspects of life related to aging or living with a disability.
The ADRC assist older adults, people with disabilities and families/caregivers in finding services and resources to keep them living well and independently.
Our services are FREE of charge and CONFIDENTIAL
The ADRC is the “Entry Point” to access Publicly Funded Long-Term Care Programs the ADRC offers:
- Options Counseling
- Long-Term Care Functional Screen
- Assess the level of need for services and eligibility for long-term care benefits.
- Helping you prepare your Medicaid application, if eligible, or explain what it means to private pay for these services.
Options Counseling
Provides information about the choices you have when making decisions about where to live, what kind of help you may need, where to receive that care and help and how to pay for it.
One-on-one consultation is available to help you think through the various options you may have in light of your situation, resources and preferences.

Youth Transition
This program is for those transitioning from school or youth programs to adult programs
There is information and assistance beginning at age 17.5 years old for youth with physical or development/intellectual disabilities transitioning into adult services. Information regarding benefits and services that may be available based on disability and functional abilities.
