Assistance Programs

These programs are administered in the Economic Support unit. Economic Support staff will determine eligibility for financial assistance program designed to assist people to meet basic living and health care needs. Each program may have income and asset limits which can vary with family size.

This is the fastest and easiest way to apply for benefits.

Apply by phone by calling Moraine Lakes Consortium at 1-888-446-1239 during regularly scheduled hours:
Monday, Tuesday & Thursday 8:00 A.M. to 4:00 P.M.
Wednesday & Friday
10:00 A.M. to 4:00 P.M.

Apply by going to 400 University Drive Suite A1, Fond du Lac during regularly scheduled office hours: Monday - Friday 8:00 A.M. to 4:00 P.M. Submit a completed paper application by mail or fax for each program you are applying for:  BadgerCare Plus or Medicaid, Foodshare, WI Shares Child Care SubsidyFamily Planning Only Services or Caretaker Supplement

For your convenience, a computer, copy and fax machine are available for your use at the following locations during regular business hours: Monday-Friday 8:00 a.m.-4:30 p.m.. Original documents mailed to the CDPU will not be returned.

QUEST Card

For information regarding how to use your QUEST card, reset your PIN, request a replacement card & monthly benefit disbursement dates visit: https://www.dhs.wisconsin.gov/publications/p1/p16088.pdf

If you have lost your card and don’t know the card number please call 877-415-5164, once it prompts you for your card number do not enter any numbers and just hold on the line to be transferred to a live worker with QUEST.
