How to request an autopsy report
Autopsy reports generated by the Fond du Lac County Medical Examiner’s Office are available by specific request, under the WI open records law. Except in the case of immediate next of kin, we require that a written request be mailed or faxed with the following information provided:
- Date of request
- The decedent’s name and date of death
- The requestor’s mailing address
- The requestor’s daytime contact phone number
- The requestor’s printed name AND signature on the request
Email requests will not be accepted.
Reports may be requested from the Fond du Lac County Medical Examiner’s Office during normal office hours, Monday-Friday 8:30 a.m.-4:30 p.m., except for official county holidays.
Requestors will be mailed a copy when the report is completed.
A charge of $90.00 will be assessed for each copy of an autopsy report, with the exception of the immediate next of kin, who will be provided a single copy, free of charge, upon request.
Checks should be made payable to the “Fond du Lac County Treasurer's Office” and mailed/delivered to the Fond du Lac County Medical Examiner's Office address at 134 Western Avenue, Fond du Lac, WI, 54935. Payment must be received before a report is mailed; however, payment should not be sent until you have been notified of the report's completion.
