In Wisconsin, confirmed or suspected outbreaks of any disease in health care facilities, including Long-Term Care Facilities, are a Category I Disease, meaning they shall be reported immediately by telephone or fax to the patient's local health officer, or to the local health officer's designee, upon identification. See tabs below for more information on reporting to Fond du Lac County Health Department.
