Caregiver Specialist Phone: 920-929-3262

The Caregiver Specialist assists individuals and families who are providing direct care to a family member or friend living at home. We can help you support them with information, assistance and resources through programs which can help them remain as independent as possible in their own home. 

  • Are you a Family Caregiver?
  • If You Answered Yes

Are you a son, daughter, relative, or a friend who:

  • Buys groceries, cooks, cleans or does laundry for someone who is unable too?
  • Helps a family member get dressed, take a shower or take medications properly?
  • Pays bills or helps manage someone’s finances?
  • Makes medical appointments and drives someone to the doctor or drugstore?
  • Stops in or calls someone on a regular basis just to make sure they are, OK?

Are you a spouse who:

  • Has taken on duties that use to be done by your spouse? (Cooking, cleaning, laundry, assisting with bathing or grooming, etc.)
  • Feels unable to leave your spouse home alone?
  • Needs to accompany your spouse to places he/she use to go alone?

If you answered YES to any of the statements from the previous tab, you are a family caregiver! 

You may think that these tasks are just things that you do for the people you love.  While that is true, don’t downplay the importance of your assistance in these areas.  Without your help, the older person may not be able to continue to live in their own home.  The “little” things that you do are crucial to the independence of your loved one.

Wisconsin Family Caregiver Support Programs

National Family Caregiver Support Program (NFCSP)

Who is Eligible?

  • Any caregiver 18 or older who provides unpaid care to a person age 60 or older, or to a person of any age who has Alzheimer’s disease or related dementia.
    • Note: The older adult receiving care must need assistance with daily living.
  • Grandparents or relatives age 55 and older who are the primary caregiver for a child under age 19 or who care for a person age 19 to 59 with a long-term disability.

Alzheimer’s Family Caregiver Support Program (AFCSP)

Who is Eligible?

Families are eligible if the three criteria's are met:

  1. There is a written diagnosis of Alzheimer’s disease or other dementia
  2. The person with dementia resides in a home setting (not a facility)
  3. The person with dementia and spouse have a gross annual income of $48,000 or less (costs related to dementia-related care may be deducted when calculating gross income)
