Fond du Lac County Transportation Program

Transportation is available for veterans needing a ride to a scheduled VA appointment at a VA Medical Center. Arrangements may be made through our office by calling 920-929-3117. This service is based on driver availability and is first come first serve so please call as early as possible.

Fond du Lac County Veteran Services Transportation Policy

This policy covers the use of the program and vehicles we use to transport veterans to scheduled medical appointments at VA Hospitals and Clinics.
1. Veteran must register with the transportation coordinator and schedule all arrangements with the coordinator.
2. Veteran must be a permanent resident of Fond du Lac County and pick-up and drop-off points must be in Fond du Lac County.
3. This transportation program is for those veterans who need assistance to get to and from their VA scheduled appointments.
4. Our drivers will transport to Milwaukee, Madison, Beaver Dam, Appleton, Cleveland and Green Bay VA facilities only.
5. Our safety policy is that our vehicles will not be on the road prior to 6am. Scheduled appointments must be made no earlier than 8am for Appleton, Beaver Dam and Cleveland, and 9am for Green Bay, Madison and Milwaukee. Additionally—The last appointment time we will transport for is 2 pm.
6. For liability reasons, our drivers are only to drive you to your appointments and bring you back home.
7. Our program is a first come, first serve program. Please call ahead for availability. Please call us early if you need transportation. Every effort will be made to accommodate you. If we are unable to provide support, alternative programs will be offered or you can reschedule your appointment to fit our scheduling.
8. Veterans are authorized to have an adult guest in the vehicle with them to assist them (no minors allowed at this time).
9. Mission Act Program (Community Care). We CANNOT transport to any location other than a VA Hospital or VA Clinic. We understand that the VA is contracting out appointments to the local communities. Unfortunately, we do not have the logistical capability to support this at this time.
10. If you have a non-appointment for a blood draw, lab or toenail clipping etc… let the coordinator know and we will schedule you to ride on a day the vehicle is already going to that location.
11. If you have the need for multiple appointments or medical procedures outside of routine appointments, please inform the coordinator of your needs so we can best serve you.
12. Surgeries: Our drivers CANNOT be your caretaker for after care. You must provide your own after care assistance. If you require a caregiver to go along with you to your appointments, please let our coordinator know ahead of time so we can make accommodations.
13. Weather issues. If there is inclement weather and it is unsafe for our drivers to be on the road, we will call you to let you know that we cannot transport. It will be your decision to either find alternate means of transportation or to reschedule your appointment for a later date. We will generally make the decision the day prior to the appointment if at all possible. Unfortunately, Wisconsin weather may call for same day cancellations.
14. Detours: Our drivers are not authorized to make detours other than emergencies or bathroom breaks, and then only to a convenience store/fuel station at highway exits. Stopping at a restaurant to get food is at the driver’s discretion. The restaurant must be in a location easily accessible on the route to and from the VA.
15. Cancellations: It is the veteran’s responsibility to contact our office as soon as possible if you or the VA cancels your appointment. If you fail to inform us, you are taking away the ability of another veteran to use that slot.

16. ALCOHOL, DRUGS (ILLEGAL), SMOKING AND WEAPONS: just like the military, we have a Zero Tolerance. Weapons are not authorized in county vehicles even if you have a permit.
17. Before calling our office for a ride, please make sure the VA has not already made transportation arrangements for you through a contracted agency such as Transtar, VA, or DAV etc..
18. Our drivers submit forms to the VA travel office informing them of who we transported. This means you are not able to collect travel allowance for trips we provide.
19. SEATBELTS: Mandatory use of seatbelts. This is a county policy as well as state law.
20. County holidays and office closures. If the Veterans offices are closed we will not schedule rides on those days.
21. If you are sick/contagious, are carrying bed bugs or lice, etc.. DO NOT schedule a transport until your condition is cleared up.
22. Infractions to these rules or improper behavior or language can result in removal of the program. Be kind, we are here to assist you.
