Adoptions

The Fond du Lac County Probate Office provides assistance to the public with filing adoptions (minor/adult) and step-parent adoptions with/without the assistance of counsel. (Fond du Lac County will allow you to file an adoption without an attorney; this is not true in other counties, check with your county before proceeding.) Our assistance is limited to procedure, we cannot give legal advice. Adoption forms are available on the Wisconsin Court System website.  Once you click on the link, hover over the Forms (top middle of page) drop-down, then click Circuit Court. Then, choose which category of forms you are searching for. Adoption records are maintained permanently on site in the Fond du Lac County Probate Office.

Copies of birth certificates can be obtained from any county's Register of Deeds Office.

Fond du Lac County's Register of Deeds office is located in the City/County Government Center.
